If densified fascia sits upstream of fibromyalgia's symptom picture, then the individual symptoms ought to have mechanical explanations rather than needing a separate theory each. Here are seven, with an honest note on how solid each one is.
1. Widespread pain
Fascia is one continuous sheet. A tissue that is continuous head to foot is capable of producing pain that does not respect the boundaries of a single joint or muscle. Well supported anatomically.
2. Pain without a lesion
Deep fascia is densely innervated with free nerve endings, but is not what routine imaging is set up to resolve. Pain generated there would be invisible on a normal scan. Well supported.
3. Morning stiffness that eases with movement
Densified connective tissue behaves thixotropically — stiffer at rest, softer with motion. That is the classic fibromyalgia pattern, and it is a property of the material. Plausible, less directly tested.
4. Tender points at tissue junctions
The classic eighteen points sit largely where fascial planes change direction or load transfers. Suggestive rather than proven.
5. Sensitivity to light touch
Superficial fascia carries sensory endings. Altered tissue mechanics there could plausibly shift how ordinary contact is registered. Mechanistic, not demonstrated.
6. Nerve-type symptoms
Nerves pass through and sit within fascial layers. Sustained compression would produce tingling and burning without damage large enough to show on conduction studies. Anatomically sound.
7. Hormonal fluctuation of symptoms
Elastography work has shown fascial stiffness varies with female hormone levels — a physical route for symptoms that track the cycle or menopause. Directly measured.
The caveat worth keeping
A model that explains everything neatly should raise an eyebrow, not settle the question. Several of these are mechanistic reasoning rather than tested findings, and none of them establish that fascia is the cause rather than a participant.
